Almost as bad as the lady in China who fell into the innards of an escalator when the cover collapsed beneath her. She had just enough time to thrust her child forward to safety before she submerged into the moving machine.
My uncle got fired from the pickle plant for putting his “equipment” into the pickle slicer while he was on break.
In all fairness, though, they fired her too. :=)
