These people are mentally ill by the definitions that a sane society would use. They are easy prey for irrational ideas like mutilating themselves or even killing themselves.
They are one of the modern day Gnostics.
Gnosticism teaches the flesh is evil and must be done away with. Transgenders do not feel right in the gender they were given, so they do away with it to make their flesh fit their mindset.
