The Romans renamed the region of Judæa (named for the Jews and the patriarch Judah) as “Palæstina” (after the Philistines), circa 135 AD when they merged it with Syria thus creating “Syria-Palæstina”.

I do not know where you got that “fog of war” story from, but if the Romans really wanted to save the Second Temple and not utterly destroy it, that’s what they would have done.

On a youtube video I watched a few years back a guy was talking about some of the things Flavius Josephus wrote about the ?The Jewish War. In his writing (If I remember it right) the guy (Josephus) said some one threw a torch into the Temple, The torch was thrown back out. But some one picked it back up and threw it back into the Temple. I suspect the Jews were responsibly for throwing the torch into the temple to set in on fire so the Romans could not have it as a treasure being the Jews at that point in the War were all ready basically losing the war big time or you could say they had lost the war but there was a few still fighting etc. in the War, and was going to fight until they were killed.
The Romans at that point probably all ready had had a few soldiers in the temple area and or in the Temple and they were probably the ones that threw the torch out of the temple, and the Jews threw it back in. But according to this guy, he just read what Josephus wrote. Josephus, in my understand of his writing, didn't say who threw the torch into the temple but their seems to be in the writing that the Romans wanted to save it as one of those wonders of the world. But it Just didn't turn out that way. That is what I would call an instance of a fog of war, call it what you want.
I will say no more or argue about it, but if you want to check it out your self you just have to read the works of Josephus.
