It's hard to make out certain words.

It's an Order of release from the military and payment for military service or something done for the military.

Said something to the extent that luitendant Gerasim Suvorov is released from military service and to be paid 500 rubles now (I think meant as one time payment) to cover his debt and paid 400 rubles until his death.
