Complete falsehoods from start to finish. I'll directly paste the requirements in a screenshot right from CDC's "COVID-19 Vaccination Program Provider Agreement Requirements":

I took the liberty of highlighting the most important part which specifically states that series adverse events MUST be reported "regardless of causality"
Now if you look at the actual law (here), you'll see that in 42 U.S. Code § 300aa–25 (b)(1)(c), it states "such other matters as the Secretary may by regulation require." Well guess what, the secretary of HHS is requiring healthcare providers to report these adverse events, REGARDLESS OF CAUSALITY.
