A lot of this problem is caused by having the bean counters in charge instead of engineers.

The bean counters only care about the next quarter, not the long run, not safety, not quality control.

They are technologically illiterate, so have no clue of the havoc they are causing.

Throw in some affirmative action hires on top of this, and what could possibly go wrong?
