They go back as far as 1910, I suppose coming from a geneologist it is so when our records become available for our future gr gr grandchildren or however far down the line it goes. Who knows many years down the road you may have a family member interested in searching his/her roots. Anyway it was not the form I heard so much about, it was very simple, and I did not answer the what is your phone # quetion. I figure I gave them what is required, now they can leave me alone.

1)How many people live at that address
2)Age of those residents
3)Sex of those residents.
