First question was “name,” second question was “do you smoke more than ten cigarettes a YEAR,” third question was “what is your cell phone number” and that is when I walked out and went back to my desk.

This year it is the “bio-metric” screening via a blood draw.

Same here. To save $25 a month each, my wife and I have to fill out a yearly questionnaire and “biometric screening”.
