Those are carried into the cabin in carry on. I know I've seen it and hand searched bags carrying millions in gems in private rooms off the main checkpoint. The x-ray can't penetrate diamonds very well and the bag has a distinct signature. The passenger (usually a jewel courier) requests a private search and is present as well as a separate witness for your safety and security.

The search is similar to a Vegas dealer leaving a table when he relieved by a new dealer.
