A universal mortality rate has never been the standard for deeming medicines unsafe for general use. I don't know exactly how the threshold is determined on whether to pull a drug off the market, but the adverse reactions to these Covid vaccines are off the charts compared to previous vaccines.
As for your apparent confidence that the FDA never makes mistakes, that is an unfounded belief. There is a long list of drugs that have been banned, even after years of use, and stating that does not make someone a conspiracy theorist.
