I had some pretty sadistic PE coaches in high school. They were amused by running everyone hard around the track. My pulse rate hit 220 BPM. I survived, but another student died. The incident was investigated. The coach was merely fired for causing the death of a student. It wasn't over consumption of water...none was offered.

A generic Google search for "student collapses dies in PE class" will yield hits all over the country. There are few records to find in the year I graduated (1973), but plenty of occurrences on a regular basis.
