Like any profession, there are people that make mistakes, many of them dumb ones. Whether he be 747 or bug smasher, all make mistakes. In aviation, it is mandatory that these mistakes be kept at a minimum.

I know this to be true.Frankly,I am sometimes surprised more don't kill each other and their pax.
