Even better, print it out, fill it out except for the signature, paste one of the "sign here" sticker-arrows that law firms use, and send it certified return-receipt with a stamped envelope addressed to the government office to which it should be submitted.

Then there's no obstacle whatsoever but his willingness to put his signature on it.

Don't forget to make documentary evidence for the media, should he try and duck it.

Yes. don't forget that he writes his speeches in long hand. He probably doesn't know how to use a computer, or the Internet.
