Is it true that Mr. Cleland's injuries aren't combat related? I read somewhere that he was playing around with a grenade and it went off. True or false?

It is true. A number of Cleland apologists have attempted to ambiguate the story and portray Ann Coulter's essentially correct characterization as a smear. A good source, including his full bio is here.
