I was active duty USAF from 31 Mar 64 until 31 Aug 85. Except for the year I was in Viet Nam, we always used letterhead paper. Letterhead or not the format always was:
The date was tabbed over about 60 spaces and was always in the form of:

dd Mmm yy

Also when referencing a member of the military you had to write the service number changed to SSN behind his name so there could be no doubt and the full name was written out.
