Why is Mrs. Kerry referred to as Heinz Kerry instead of just Kerry? Or Teresa Kerry? Is it in a pre-nup d'ya suppose?

What I want to know is what state did Therazzza claim as residency? According to her 1040 it was prepared in PA. Makes me wonder if she used PA as her Residency, since a copy of her MA Tax Forms are not on the Kerry website. Not to mention the I believe the tax rate is lower in PA. Or did she claim MA this year and in past years Claim PA.
