QUESTIONS:
Was the rider in control, or had to horse bolted?

What the Hell was the rider doing anywhere near vehicle traffic? Not good, unless you are in a parade.

My brother lost control and did not slow the horse down.

We live on a dirt road and have horse riders going by every day.

We don't know, I'd assume at least a momentary loss of control. In fairness on your second point, I grew up in the country and used to ride all over our area, along country roads to the points where we could ride off into the woods. Sometimes we rode right into town. Was a small town, but town all the same.

I've been considering whether I would do that again recently, because there are some trails not too far from my home that I'd like to check out. We don't yet have a trailer and can't go on trail rides whenever we like.

As a teen, I had fewer fears.

I agree. Sounds like another case of a loose nut behind the saddle horn.
