Doesn't take much to be a citizen, if this test is an indication! - But, seriesly, on question one, the Press at least thinks it elects the president - I think the fellow who thought up that question has a sense of humor.

They ARE called STATES. (Well, maybe "Commonwealth," but certainly not "colony.")

The question was, "8. What kind of government does the United States have?"
The stated correct answer was, "Democratic" but that is inaccurate, IMO.
The correct answer should be, "Representative Republic."

Notice when it asked to name a Right guaranteed by the Constitution the answer was:
Freedom of: speech, press, religion, peaceable assembly, and requesting changes in the government
Correct, but what about the Right to bear arms, the 2nd Amendment no less???

I got 20/20. I betcha the question about the "form of government" would throw a lot of natural-born adult citizens, as one of the possible answers is "democratic". (Hint: that's not the correct answer!)
