Does Fox know he's lying or is he naively repeating a script fed to him by manipulative Democrats who care nothing about him or his illness?

You bet Fox knew what he was doing. Even if he was not sick he's still a flaming liberal. I have known two people who had Parkinson's Disease and one of them died from it, but I never saw either one of them jerk around like Fox was doing. He was on Boston Legal right before this ad aired and he didn't shake that bad. I'm one of the ones who thinks he was adding to it so that it would make him appear sicker than he is, and to make President Bush look more evil for not signing it the legislation.
