Did you read the photographer's web site? She states that these purple heart photos are meant to highlight the terrible human toll of the war. I'm not sure that this is even a real photograph.

My guess is yes, it's a staged photo, maybe even two photos put together. This is a horrible web site, exploiting wounded soldiers. She's got pictures that look like they're from Guantonomo, other pictures under a file she calls, the Taliban. This photographer is probably a friend of Crazy Cindy Sheehan.
