I suspect there is film of McCain receiving very "special treatment" from his captors -- while receiving the "special medical" treatment he received.

It's hard to tell exactly what you mean by this, but I've heard from at least one other former guest of the Hanoi Hilton that when McCain came back from "interrogations", they would have to wipe his backside for him because he was too badly beaten to do it himself.
I am by no means a fan of John McCain; even if his behavior as a POW were universally acclaimed to have been exceptionally meritorious, he's covered himself in plenty of shame since then.
I myself went through the USAF SERE school in June of 1971; the POW Camp phase lasted two days. During that period of time, one of the general rules was that the "guards" couldn't do anything to us that would cause permanent injury. Even so, those two days alone were unpleasant enough that I swore I would never be "taken alive".
I can't for the life of me fathom how those guys managed to survive those conditions for years without becoming vegetables.
