well yeah you have the taxing power of the entire federal government to service that part of the population. Nevertheless, it is not going to give people the right incentives for healthier lifestyles.

Medicaid doesn't provide care. It pays (VERY POORLY) the bills. The only reason most Healthcare systems will see Medicaid patients is because the State holds a gun to your head. If you want to do buisiness in the state you will take X number of Medicaid patients on board. Example, if I see a Medicaid patient in the ER we get $30. Doesn't matter it it's a nothing, or an Acute MI that ends up on a balloon pump in the ICU.
