This may have already been suggested, but how difficult would it be to simply put up a curtain or a partition so they can scrub up privately? Problem solved.
Of course, accommodating them in this manner may simply lead to more demands, but the notion that they shouldn't scrub up to the elbows is ridiculous. Patient safety trumps any 'religious' concern. Otherwise, find another job or go back to the 3rd world islamic hellhole from which you escaped.

Such a simple solution, entirely accommodating, yet not enough for them. It's apparent that female modesty has nothing to do with their demands. As you noted, it's about flexing muscle and making more and more demands.

The rule is no sleeves below the elbow at any time. Skin is much easier to wash every twenty minutes than cloth. Rolling up dirty sleeves only to roll them down again does nothing to prevent spreading disease.
