Have you scrubbed an abdominoperineal resection for low rectal or anal carcinoma. It is fairly brutal. When you finally remove the rectum it looks like a bomb went off. (Of course a radical hemmorrhoidectomy looks bad also)

The hemipelvectomy is extraordinary in its applied trauma also.

Haven’t seen one lately. I got called to consult on one twenty years ago when the patient developed coagulopathy intraoperatively. Not sure what caused it but no matter what we did he wouldn’t stop bleeding. Not a good outcome.

Some of the head and neck cancer procedures are pretty mutilating, too.
