Not necessarily true. My wife got one two weeks ago with one. Her original was lost and all we had was the certificate.

This certificate would not get you a passport because it is not certified or has no official notary. It would be declined.

Yes, but it had a signature and it had an appropriate stamp on it. This one has neither. There is no way it could get a passport. They would reject it in a nanosecond.
