Sometimes genotype and phenotype do not match. With sufficient prenatal testosterone (not a completely uncommon occurrence), genetic females will look identical to normal boys.
To an extent yes, but it's been know for some time that the mother's internal environment can affect the probabilities. The sperm carrying the Y and X chromosomes are not identical, even under an ordinary microscope. They react differently to that environment, and they perform differently as well. So there can be a timing effect between time of ovulation and time of insemination, but of course that is a joint effect of both parents.
