Yes. The form looks real to me, but the filling it out doesn’t.
Forms do change, even in Indo, but it just doesn’t “feel” right to me. I’d want to exam it closer.
If the original is still extant, I believe the age and composition of the ink can be checked. Beats the heck out of a fax of a Word doc.
