We don’t really know if he feels empathy for others. He may feel empathy but just ignore it. He also doesn’t have the conduct problems usually found with anti-social personalities.
I don’t like diagnosing people without having met them, but pretty much everyone agrees that Hitler was anti-social. Stalin was more of a paranoid personality than narcissistic IMO.

I just thought the comparison was worth mentioning as it put Obama’s diagnosis in some pretty infamous company.
