Per the suplementary article I posted at number 2 or 3 he didn’t survive a contract per se, in that the Joe Pesci character try to kill “Lefty” without authorization, which lead to his own death.

If there is a contract probably fairly low, unless you go deep into hiding.
The reason Lefty survived was that there wasn't a contract, the brother's were freelancing. Had there been a contract the killers would have kept trying until they got it right. On the other hand if you freelance you had better do it right and quietly the first time. Drawing attention to the organization is a big no-no in the mob. A botched freelance hit, as mentioned in the story, has a high probability of getting you dead.
