I think a 5th official in a "replay" booth could be useful for a limited set of circumstances could improve the situation greatly without impacting the flow of the game.

The limited criteria should remove as much human error as possible without stepping on the actual judgment of the officials. I would think the following criteria would be easy enough to review quickly and meet the above guidelines:

All three of those cases could be reviewed before the ball was put back in play -- they're very straightforward and the threshold of "proof" can be defined highly enough that we don't have NFL-style 2+ minute reviews.

In any event, suspensions due to accumulated cards in a tournament like this (or in league play, for that matter) should allow players to appeal discipline for the purposes of suspension. The tournament officials or league should likewise have the ability to issue discipline outside of the match for blatant fouls that were not called on the field
