For one thing it is clear that it was completed using TWO DIFFERENT TYPEWRITERS.
Examining the information pertaining to each typewriter it looks like a nurse/clerk completed everything that would apply to any birth at that hospital, and later a different nurse/clerk completed everything related to just this birth.
That's quite logical.
Plus, Ann herself is given as the source of all the information ~ and she tried, but didn't quite make it on working out where the supposed baby-daddy came from. He, like the rest of his family, were from Mombasa in the East African Protectorate. That city is also the capital of Kenya. Now that's easy enough for folks who live in a federal society to figure out, but probably not at all comprehensible in Hawaii ~ 'cause they are not different islands eh.
Ann didn't do the typing BTW, just gave the information.
BTW, I have NOTHING that I have to backtrack on since the first day the issue came up ~ when the Democrats went after John McCain!
There's one slip up in this ~ there's a shift back to the ORIGINAL TYPEWRITER just before this document was put into the book. Can you see where that is?

I suspect that just moments after I mentioned, again, the problem with Mombasa being involved with two different countries in 1961, they fixed the Protectorate/Kenya dichotomy. There's no way Stanley Ann would have fixed it with Kenya, East African Protectorate ~ should be Mombasa, East African Protectorate ~ so, guys, the CIA still hasn't quite got it right, but that doesn't men he wasn't born in HI ~ somewhere.
BTW, the reason the hospital info is wrong is they did the original typing (which is why there was some original document material lying about) but alas the other hospital where he was really born ran out of forms and borrowed some.
Typically when that happens they'll overwrite thewrong stuff. This time they didn't. Those crafty CIA text analysts and forgers are sometimes stupid. (/s)
