I agree: the more the market values are in medical care, the better that care will be. We need to destroy Obamacare.

You don't even have to make an offer. Our hospital-attached physician was asked how much discount if we cut a check immediately? Answer: 30%.
Brought daughter into emergency walk in clinic. They took care of her swing set gash immediately. While they were doing it, I asked, how much discount if I pay with a debit card right now? Answer: 30%. The desk lady didn't act like she was asked anything out of the ordinary.
Rush Limbaugh has some chest pains while golfing in Hawaii. On the way into extensive medical treatment, he asks: how much discount if I pay you right away? Answer: 30%.
My conclusion is that if you ask them first, the standard discount is 30%. That strikes me as fair. Maybe I could have wheedled out another 5% or so, but I am inclined to accept reasonable offers for honest service made in good faith.
