My mistake, I meant the National Center for Health Statistics and the National Vital Statistic Division
In 1961 both were part of the Dept. of Health. Look at the cover pages of the 1961 Vital Statistics of the United States, Natality

Also look on page 5-4 at Figure 5-1 of the Technical Appendix. There is a copy of the Standard Certificate of Live Birth. States like Hawaii modified the standard certificate but much of the information remained the same.
Zullo says twice that coding was a Federal requirement and he never mentions a separate Hawaii coding system.
Can you specifically point to where Zullo even mentions the tables used in the video?
