I think the future of quality US medical care will be offshore.

The best and brightest will set up facilities in the Caribbean (outside the jurisdiction of the fedgov) that will serve the top 5%.

It will be like private jets vs the “human cattle car” of commercial aviation. Or Baraq Obama sending his kids to Sidwell while the peons head to DC schools.

Hey, you stole my line! B-) Seriously, I've said the same in the past. If I had the capital and Bammy is re-elected (shudder the thought, this is just a brain game for now and hope it stays that way), I'd get me a pilot's license and an old WWII Catalina PBY flying boat and fly people down from Pittsburgh to the Honduras or wherever. If I was really rich, I'd help those who could not pay. Better, yet, I'd use an old Lockheed Electra.

Some go as far as the Phillipines for good care.
