Ah, no /s tag but I'll assume you know that's exactly what happened in 1999:

"Metric mishap caused loss of NASA orbiter."
"NASA lost a $125 million Mars orbiter because a Lockheed Martin engineering team used English units of measurement
while the agency's team used the more conventional metric system for a key spacecraft operation ..."

Not to worry though - it was replaced with another $125 million from the taxpayers' wallets.
