I don't think anybody's going to initiate a nuclear war. Desperate people, who are losing, do desperate things. And they would be doing it to preserve themselves and their stuff, not to destroy it.
Although the Iranians,or even some faction of the Pakistanis, might use nukes at the outset, out of Mussleman religious zealotry.

Often, but far from always. Sometimes it's over ideology, who's got the biggest.. well you know, all sorts of things. WW-I was only marginally about getting territory, and the territory had already been gotten by the Austria-Hugarian Empire. It was more about the "big.. " factor. It was a damn bloody war that wrecked the economies of most of the participants, and the political systems of others.b Similarly the US Civil war was more about ideology than getting stuff.The Civil War was fought over territory. If the Confederacy had abolished slavery and decided to secede anyway, would Lincoln have allowed them to leave and ended the war?

Although the Iranians,or even some faction of the Pakistanis, might use nukes at the outset, out of Mussleman religious zealotry.That only works against a non-nuclear opponent. According to Muhammad, any Muslim who dies trying to kill the infidel goes to paradise, along with 72 of his favorite friends and relatives. How many Muslims have taken that literally? Tens of thousands out of 1 b people - literally 1 out of 10K Muslims. Ultimately, these people embrace the forms and practices of Islam, but don't really believe. Even hardcore jihadists like bin Laden don't really love death as we love life - they're men of little faith who claim to do so in order to egg on their underlings to greater efforts so the leadership can use them to achieve its goals. And still the jihadists carrying out suicide operations need to fortify themselves with hard drugs to follow through with their operations.

If one alternative is a long drop on a short rope, many things are possible as the other alternative. The Iranian leadership may feel they are sufficiently deeply buried to survive a counterstrike. The Soviet leadership at one point thought they were, and they may have been correct.
