It looks like a typewritten document (on an actual typerwriter)
It DOES NOT look like an official federal government form, and there is no form number on it.
WHO WROTE IT?
Wait until they get their claws into your healthcare .... they'll be demanding a list of everybody you've ever had sex with (important for health history purposes), what kind of sex (unless you are 'gay' of course - that would be discriminatory), and what you were thinking while the sex act was in progress.
The answer would be this: MYOB.
And their answer will be .... sorry, due to incomplete information, request for tests/procedures, perhaps even healthcare ... denied. I am not putting ANYTHING out of the realm of possibility with these thugs - the object is to “control”, whatever it takes.

They supported targeting of “hate” groups like the Tea Party. One said it was a tactic that has been be used to good effect in the past against other groups, and didn't see why it made the news.
He is convinced that the media hates Obama now, because the IRS audits were totally justified.

If we can't get a conservative to run in 2016, I'll say that we are done.
Young liberal idiots say things like that never considering that a double edged sword cuts both ways. Disregard their ignorant ramblings as youthful inexperience and lack of wisdom. Treat them as the children that they really are as they are people unworthy of respect.
