Smokers have been paying higher premiums for ages.

But smokers die earlier so they don't require as much care as people who continue to drink booze.
Or people who take baths and fall down, or fall riding bicycles, etc.

So they all should be charged more according to your standards.
So after you dissect everyone's lifestyle, who gets charged less?
