CAn someone explain to me how Obama’s “gramps” hand is on his shoulder? His arm is pointing straight down at the shoulder.

The arm isn’t hanging straight down. The arm is turned outside, it goes down at a steep angle to the elbo, and is then raised up behind Barry and on over to his shoulder.
It was probably a last moment decision, and didn’t get to the resting location before the shot was taken.
