In the last 5 mos I’ve filled out more health forms than I can count. Not one of them asked for this info.
My solution to filling out medical forms is to keep up to date lists of my medications and surgical procedures on my computer. Any time I go to a doctor who doesn’t have that information, I print out both lists. Saves A LOT of time going through the initial paper work.
Not all docs are using the new forms, why waste what you have on hand? They are a business and those forms cost bucks.
What I’ve seen a lot of is NO NEW MEDICARE PATIENTS.
