I had a former friend, liberal axe, and she insisted her psychologist husband be called Dr. I always cringed at it since he was a wine swishing bearded psychologist trouble maker

Not really.

It is traditional that persons tend to be addressed by the highest degree they attained or the highest position (governmental) they held.

Recall, 'don't call me Madam! I am a Senator!'?

Earning a PhD is (used to be) no small matter. The same with JD and M.D.
