Don’t buy the insurance and instead use the huge cost the “insurance plans” would have cost to go to the hospitals of ones choice...the hospitals may very lower their costs for those who pay cash directly!
That’s kind of how it works in Germany. They put you to the front of the line when you pay cash and it is not expensive.
