My brother was an electrical engineer on the three Columbia dams. I remember him telling me that one got poured ....backwards. That is....mirror image. Big snafu. I imagine a lot of drillin’, fillin, and whatelse was needed to wire and plumb it. I’m just guessin’.....but I’d say it’s the prime suspect for going boink.
