Many Americans get subsidies from the government. The 49.5 million on Medicare get, on average, three times in benefits than they contributed to the system. 40% of all Medicare expenditures come from the General Fund.

You left out Medicaid/CHIP, which has 71 million on the rolls.

Under Obamacare 16.4 million uninsured gained coverage under the ACA since 2010. There were 10.8 million Medicaid and CHIP enrollments since October 2013. The Obama administration said Tuesday that 11.7 million Americans now have private health insurance through federal and state marketplaces, with 86 percent of them receiving financial assistance from the federal government to help pay premiums.
