Under a republican form of government, is it legitimate or lawful to force doctors and patients into a contractual relationship neither wants? California claims to be a Republic?

I understand the Courts would probably not touch this question with a 10-foot pole, but for the future of this country, we need to be having this kind of discussion. Also, we need to be talking about how the world view of some prospective immigrants adds or takes away from the ideal. Same with the effect of government schooling. California illustrates what happen to the entire US if we admit a large number of what amounts to uneducated peasants, and allow them to vote. We can still appreciate aspects of the different cultures without making them all citizens.
