As usual, you have to
go to the UK to get anything substantive on this story. Lots of biographical information and photos at the link. Sounds like she had a decent career up to now (including ironically working many years as safety engineer and emergency management expert). One of her jobs was at the Johnson Space Center. This fatal mistake will ruin all of that.
Journalism is pretty much dead here in America. Have to go to UK for American news.
