"I'm not raising this again: fix it, or I will [have] no choice but to find a way to penalize people. Not my preference, but I won't have my instructions ignored," he added.

This is a sign of a person with no leadership skills. It appears he also has staff that can't follow simple instructions. In the real world you tell someone to fix a mistake. You don't ask questions like he did and you don't mention consequences, as your subordinates would already know them.

Leaders and professionals do not write negative memos to their whole staff.

They identify the person and the issue and talk to that person off line.

Di Blasio is clearly a jerk.
