What the heck? The most advanced warships/technology in history...and two collisions within months.

Like I said last time — there are more admirals than ships.
This means that the navy is packed with dead weight stick-in-the-mud admirals. How many of them came up with a plan to prevent this from happening again?
Captains need to be checked with random, ‘out of the blue’, collision tests. It would be easy to simulate them trhough some kind of drone technology.
