The workers harness was most likely old and rotted out.
Just had a safety guy screaming about old harnesses last month, this happens a lot (harness failure), and it’s a damn shame.
At the time of failure it doesn't appear the crane was doing anything other than providing a safety line for the worker. That crane doesn't move when the bridge collapses from what I can tell. I am guessing the crane was there to aid in building the tower and other general duties as required.
A couple days old, but here is confirmation they making adjustments to tensioning rods when the bridge failed.
