Does any IG report recommend specific individuals be prosecuted?
The IG is like a company auditor. Reports are shared with chain of command, who take action as they deem appropriate.

Even reports on what seem like pretty heinous crimes almost all end with a statement like, “Criminal prosecution of the SA was declined.”

I think it recommended that five people be “investigated”; nothing more.
Since they can't prosecute the only thing they can do is make a criminal referral. They recommended that McCabe be fired, they did not make a criminal referral which I am sure they could have but chose not to. They are after all part of the deep state.
IG doesn’t recommend prosecutions.
